Magic Liquidizer Responsive Form and Magic Liquidizer Responsive Table are two awesome WordPress plugins which upon activation turns HTML table and forms responsive.
Install and activate these two plugin and then visit your admin area dashboard Magic Liquidizer Lite -> Form/Table, specify your form/table class or id (if required) and then tick the checkbox.
You can also define a custom form and table breakpoint e.g. 480px, 720px, 840px, or 960px. Finally, when you are done with options, save changes and that’s it.
